Welcome to the National Assessment Agency (NAA) website for national curriculum test forms. This site provides online application and notification forms, and guidance on their completion.

In addition downloadable templates to support national curriculum assessment and reporting arrangements can be found here for KS1, KS2 and KS3.

To make online applications and notifications please enter your test forms username and password.
	Your username and password are provided to you by your school's superuser.The Password is case sensitive, 8 characters.  Please remember to use your most recent username and password.

If the headteacher has any account problems, including forgotten password and locked account, please contact your local authority superuser.
