National curriculum test forms

The following forms and templates can be downloaded, completed on your word processor or printed and completed by hand. Please select the form you wish to use and follow the instructions below.

The forms are in two formats:

PDF (portable document format) file. You can print the form, complete it by hand and return it following the instructions on the form. You need Adobe Reader to open a PDF. Download Adobe Reader here for free.
MS Word file. Save it onto your computer, complete it using your word processor, print and follow the instructions on the form.

Guidance for each form is provided in PDF format.

This template has been designed to help headteachers write a report on pupils working at the levels of the tests but unable to access them.
